Club Registration Packet

This form/packet should be completed by PARENTS AND PLAYERS to register to practice/play with our club. Registration Packet contains all the forms necessary to join the club and play rugby.

If you have not already submitted a physical form to SCASD this school year, a completed physical form is required.

 

RugbyPA/USA Rugby Registration

 

You must register online at RugbyPAAll teams, coaches, and players are required to be members of RugbyPA and USA Rugby.  

SCASD Fitness Center Permission Form

 All State High Fitness Center club members and athletes must be certified and have a current year Fitness Center Parent Permission Form. All students are offered certification during PE class each year. There are also certification classes offered occasionally after school for a fee. The Parent Permission form must be returned directly to fitness center.

Volunteer Clearances, If Applicable

All adult/coach/parent volunteers are required to submit Volunteer Requirements .
